Aesthetic & Primary Care Wellness Membership Types:

Tox Club:

Cost is $20 per month but gives you $3.50 pricing per unit when getting Dysport which normally runs at $5 per unit.

Silver Wellness Membership: 

$99 per month: Pick one treatment a month-small laser hair removal, 4 vitamin injections one a week, small tattoo removal, or one happy b vitamin hydration.

Gold Wellness Membership:

$149 a month. Pick one treatment per month. 6 vitamin shots, Medium Hair removal, 2 Radio Frequency Treatments per month, Medium Tattoo removal, IV hydration (500 ml with 3 vitamin combo)

Platinum Wellness Membership:

$199 a month Pick one per month: General Wellness Labs (CBC, CMP, Lipid Panel, Vit D, Vit B12, TSH (limited to every 6 months), One large tattoo removal, one large hair removal, 8 vitamin shots, Custom IV with 1000 ml of fluids (Myers or Executive), One IPL photo facial

VIP Primary Care Membership:

$100 per month per person ($25 per child per month if adult is also a member) Cost includes Labs every 6 months, Unlimited office visits, telehealth visits, Any test ablet to do in the office, referrals, Xrays ordered. Access to provider at all times. Hour long appointments. People use this type of plan if they don’t want to use their high deductible plan or if they have no insurance at all. We practice preventative medicine instead of reactive medicine where you don’t want to go to your provider until it is too late.

Regular Insurance:

We are out of network for most insurances companies at this time but we are actively in the credentialing process and working on this now to change this. With this we can create a super-bill that will need to be paid in advance, and then you can get reimbursement from your insurance company if you choose so. We can get appointments, labs, and imaging at a lower cost if you are wanting to do the membership.
